Introducción: el virus de la inmunodeficiencia humana sigue siendo de los principales problemas para la salud pública a nivel mundial ya que es causante de muertes a nivel global. La generalización del test del VIH en la población es una forma de estrategia de salud pública. Objetivo: determinar la frecuencia de la infección por el VIH en pacientes que acuden a urgencias en el período de marzo 2021 a octubre 2023. Metodología: estudio observacional, descriptivo, fueron incluidos pacientes adultos internados en el servicio de urgencias durante el periodo de marzo 2021 a octubre del año 2023. Para el cálculo del tamaño de la muestra basada en una proporción esperada de 0,1 (1 %) y una amplitud de 0,10 (precisión), nivel de confianza de 95 % lo que arrojó un tamaño de 974 pacientes. Los datos fueron procesados en el paquete estadístico EPI Info (Atlanta- CDC). Resultados: se incluyeron en el estudio 976 pacientes, la frecuencia de la seropositividad del virus de la inmunodeficiencia humana por test de ELISA fue del 12,8 % (n = 126) la media de edad de los pacientes positivos fue de 38,4 ± 13 años, en cuanto a los factores de riesgo 18,25 % (n = 23) refirieron consumo de drogas ilícitas; 16,66 % (n = 21) tatuajes; 11,90 % (n = 15) antecedente de transfusiones de hemoderivados; 7,14 % (n = 9) piercings. Conclusión: la frecuencia de la seropositividad del VIH por test de ELISA fue del 12,8 % en pacientes adultos que acuden a urgencias por cualquier razón.


Introduction: the human immunodeficiency virus continues to be one of the main public health problems worldwide and is a major cause of death. The generalization of HIV testing in the population is a public health strategy. Objective: to determine the frequency of HIV infection in patients who presented to the emergency department from March 2021 to October 2023. Methodology: this was an observational, descriptive study; adult patients admitted to the emergency department from March 2021 to October 2023 were included. For the calculation of the sample we used an expected proportion of 0.1 (1 %) and an amplitude of 0.10 (precision), confidence level of 95 %, which yielded a size of 974 patients. The data were processed in the EPI Info statistical package (Atlanta-CDC). Results: 976 patients were included in the study, the frequency of human immunodeficiency virus seropositivity by ELISA test was 12.8 % (n = 126), the average age of the positive patients was 38.4 ± 13 years old, regarding risk factors, 18.25 % (n = 23) reported illicit drug use; 16.66 % (n = 21) tattoos; 11.90 % (n = 15) history of blood product transfusions; 7.14 % (n = 9) piercings. Conclusion the frequency of HIV seropositivity by ELISA test was 12.8% in adult patients who come to the emergency room for any reason.

RESUMO Objetivo compreender as perspectivas e desafios no cotidiano de pessoas após a descoberta do viver com VIH em Bissau, Guiné-Bissau, tendo em vista diferentes contextos de vulnerabilidade. Método estudo exploratório-descritivo, que utilizou entrevista semiestruturada com 16 pessoas vivendo com VIH, acompanhadas em um hospital de Bissau. Empregou-se técnica de análise de conteúdo temática. Os relatos dos participantes foram analisados a partir de duas categorias empíricas: A descoberta, os impactos e os desafios de viver com VIH; e Experiência com o antirretroviral: recomeço e perspectivas. Resultados sinalizaram que os desafios iniciam com a revelação do diagnóstico que, geralmente, desperta uma diversidade de sentimentos e comportamentos. O estigma e a discriminação estimulam a adoção do sigilo sobre o status sorológico, resultando na fragilidade de suporte emocional no enfrentamento à soropositividade. A terapia antirretroviral foi vislumbrada como esperança para o enfrentamento da doença. A vulnerabilidade social foi a dimensão que mais se destacou, e violações dos direitos humanos foram constatadas. Conclusão e implicações para a prática o estudo permite compreender as perspectivas, desafios e vulnerabilidades de pessoas que vivem com VIH. O viver com VIH merece atenção especial por parte dos profissionais de saúde que atuam no cuidado dessas pessoas, destacando-se como contribuição a relevância de um cuidado de saúde integral, em que a ética e a subjetividade estejam presentes.


RESUMEN Objetivo este estudio cualitativo tuvo como objetivo comprender perspectivas y desafíos en la vida cotidiana de las personas que viven con VIH en Bissau, Guinea-Bissau, considerando diferentes contextos de vulnerabilidad. Método estudio exploratorio-descriptivo, que utilizó una entrevista semiestructurada con 16 personas que viven con el VIH, seguido en un hospital de Bissau. Se utilizó la técnica de análisis de contenido temático. Los informes de los participantes se analizaron a partir de dos categorías empíricas: El descubrimiento, los impactos y los desafíos de vivir con el VIH; y La experiencia antirretroviral: un nuevo comienzo y perspectivas. Resultados los resultados indicaron que los desafíos comienzan con la divulgación del diagnóstico, que generalmente suscita una diversidad de sentimientos y conductas. El estigma y la discriminación fomentan la adopción del secreto sobre el estado serológico, lo que resulta en un apoyo emocional débil para hacer frente a la seropositividad. La terapia antirretroviral es una esperanza para hacer frente a la enfermedad. La vulnerabilidad social fue la dimensión que más se destacó y se encontraron violaciones a sus derechos humanos. Conclusión e implicaciones para la práctica el estudio permite comprender las perspectivas, los desafíos y las vulnerabilidades de las personas que viven con el VIH. Vivir con VIH merece especial atención por parte de los profesionales de la salud que actúan en el cuidado de estas personas, destacando como aporte la relevancia de la atención integral en salud, en la que la ética y la subjetividad están presentes.


ABSTRACT Objective to understand perspectives and challenges in the daily lives of people after the discovery of living with HIV in Bissau, Guinea-Bissau, considering different contexts of vulnerability. Method an exploratory-descriptive study conducted through semi-structured interviews with sixteen people living with HIV attending a hospital in the city of Bissau. We used the thematic content analysis technique. Participants' reports were analyzed following two empirical categories: Discovery, impacts and challenges of living with HIV; and Experience with antiretroviral therapy: new beginning and perspectives. Results the results indicate that the challenges start with the disclosure of diagnosis, which arouses a diversity of feelings and behaviors. Stigma and discrimination encourage the adoption of confidentiality about serological status, which increases the fragility of emotional support in coping with seropositivity. Antiretroviral therapy was seen as a hope for coping with the disease. Social vulnerability was the dimension that stood out the most, and human rights violations involving people living with HIV were verified. Conclusion and implications for practice the study makes it possible to understand the perspectives, challenges and vulnerabilities of people living with HIV. Living with HIV deserves special attention from health professionals who work in the care of these people, highlighting as a contribution the relevance of comprehensive health care, in which ethics and subjectivity are present.

ABSTRACT Objective: To evaluate the seropositivity rate of rapid tests for HIV, syphilis and hepatitis B and C among transvestites and transgender women (transfeminine persons) inmates in the metropolitan region of Rio de Janeiro, analyzing the results based on sociodemographic, prison profile and access to health technologies to prevent sexually transmitted infections (STIs). Methods: Cross-sectional census-type study carried out with transfeminine in eleven male prisons in Rio de Janeiro, between the months of April and June 2021. Results: The detection rates found were 34.4% for HIV, and 48.9% for syphilis, and 0.8% for type B and C hepatitis. Seropositivity for more than one infection was verified in 25.4% of participants, and HIV/syphilis was the most prevalent. An increase in the level of education (p=0.037) and having a steady partner in prison (p=0.041) were considered protective factors for STIs in this population. Difficulties were identified in accessing STI prevention technologies, such as male condoms, lubricating gel, rapid tests, and prophylactic antiretroviral therapies for HIV. Conclusion: HIV and syphilis seropositivity rates were high, but within the profile found in this population in other studies inside or outside prisons. The data found indicates the need to incorporate effective strategies for access to health technologies for the prevention of STIs. The scarcity of scientific publications containing epidemiological data on STIs in the transfeminine prison population limited deeper comparisons of the results obtained in this study.


RESUMO Objetivo: Avaliar a taxa de soropositividade dos testes rápidos para HIV, sífilis e hepatite B e C entre travestis e mulheres transexuais (transfemininas) privadas de liberdade na Região Metropolitana do Rio de Janeiro, analisando os resultados diante do perfil sociodemográfico, prisional e acesso às tecnologias de saúde para prevenir infecções sexualmente transmissíveis (IST). Métodos: Estudo transversal do tipo censitário realizado com transfemininas em 11 prisões do Rio de Janeiro, entre os meses de abril e junho de 2021. Resultados: As taxas de soropositividade encontradas foram de 34,4% para o HIV, 48,9% para sífilis e 0,8% para as hepatites do tipo B e C. A soropositividade para mais de uma infecção foi verificada em 25,4% das participantes, e HIV/sífilis foi a mais prevalente. O aumento no nível de escolarização (p=0,037), e possuir parceiro fixo na prisão (p=0,041) foram considerados fatores de proteção para as IST nessa população. Foram identificadas dificuldades no acesso às tecnologias de prevenção contra IST, como preservativo masculino, gel lubrificante, testes rápidos e terapias antirretrovirais profiláticas para o HIV. Conclusão: As taxas de soropositividade para o HIV e sífilis foram elevadas, mas no perfil encontrado nessa população em outros estudos dentro e fora das prisões. Os dados encontrados indicam a necessidade de incorporar estratégias efetivas para o acesso às tecnologias em saúde para a prevenção das IST. A escassez de publicações científicas contendo dados epidemiológicos sobre IST na população transfeminina em situação de prisão limitou a realização de comparações mais profundas dos resultados obtidos neste estudo.

ABSTRACT

Objetivo: discorrer sobre as condições de vida das pessoas vivendo com HIV em um município do estado do Rio de Janeiro, Brasil. Método: estudo do tipo transversal, descritivo, de natureza qualitativa. A coleta de dados foi através de entrevistas, sendo os achados submetidos à técnica de análise de conteúdo. A amostra foi composta por pessoas vivendo com HIV que eram abordados no momento que compareciam ao serviço de assistência especializada, sendo definida uma amostra de 20 participantes por técnica de saturação. Resultado: emergiram categorias com base nos temas abordados na entrevista relacionados à qualidade de vida (condição de alimentação, atividade física, lazer, religião e satisfação com a vida sexual) e à experiência de viver com HIV. Conclusão: as pessoas vivendo com HIV, em tratamento antirretroviral, podem usufruir de boas condições de vida tendo como principal fator impeditivo o medo de vivenciar situações de estigma e discriminação.


Objectives: investigate the implications for the life conditions of people living with HIV in a city of the State of Rio de Janeiro, Brazil. Method: cross-sectional, descriptive, and qualitative study. Data collection was conducted through interviews, and the findings were subjected to the content analysis technique. The sample comprised people living with HIV that were approached at the time they attended the specialized assistance service, and a sample of 20 participants was defined by saturation technique. Result: categories emerged based on the topics addressed in the interview related to quality of life (food condition, physical activity, leisure, religion, and satisfaction with sexual life) and the experience of living with HIV. Conclusion: people living with HIV in antiretroviral treatment can enjoy good living and the preventing potential factor is the fear of experiencing situations of stigma and discrimination.


Objetivos: conocer las implicaciones para las condiciones de vida de las personas que viven con el VIH en un municipio del Estado de Río de Janeiro, Brasil. Método: estudio de tipo transversal, descriptivo, de naturaleza cualitativa. La recolección de datos se realizó a través de entrevistas, y los hallazgos fueron sometidos a la técnica de análisis de contenido. La muestra se compuso por personas que viven con el VIH que fueron abordadas en el momento en que vinieron al servicio de asistencia especializada, y se definió una muestra de 20 participantes por la técnica de saturación. Resultado: emergieron categorías basadas en los temas abordados en la entrevista relacionados con la calidad de vida (condición alimentaria, actividad física, ocio, religión, y satisfacción con la vida sexual) y la experiencia de vivir con VIH. Conclusión: las personas que viven con el VIH en tratamiento antirretroviral pueden disfrutar de buenas condiciones de vida y salud, y el principal impedimento y el temor de experimentar situaciones de estigma y discriminación.

Objetivo: conhecer os níveis de depressão e sintomas depressivos em angolanos com HIV. Método: estudo descritivo e misto, com estratégia exploratória sequencial, em período pré-pandêmico em um hospital público de Angola. Realizou-se entrevistas semiestruturadas, com aplicação do Inventário de Depressão de Beck. Para fins de análise, considerou o conceito de depressão a partir da classificação internacional de doenças. Resultados: prevaleceu depressão leve a moderada para ambos os sexos e estado civil. No G1, a maioria além da tristeza ou infelicidade não conseguem suportar a angústia, alguns verbalizam ter consciência de que os choros não os levará a ter vida anterior desta realidade. Agora se sentem irritados o tempo todo e não sentem mais prazer nas coisas como antes. No G2, expressaram ideias e sentimentos suicidas, estavam desanimados quanto ao futuro, percebem-se fracassados mais do que uma pessoa sem a doença, e adiam a tomada de decisão mais do que o costume. Conclusão: embora não tenha prevalecido formas severas (graves) de depressão, a ocorrência de sintomas negativos em variadas esferas da vida pode evoluir, especialmente no contexto angolano.


Objective:to know the levels of depression and depressive symptoms in Angolans with HIV. Method: descriptive and mixed study, with sequentialexploratory strategy, in the pre-pandemic period in a public hospital in Angola. Semi-structured interviews were carried out, with the application of the Beck Depression Inventory. For analysis purposes, it considered the concept of depression from the international classification of diseases. Results: mild to moderate depression prevailed for both sexes and marital status. In G1, the majority, in addition to sadness or unhappiness, cannot bear the anguish, some verbalize being aware that crying will not lead them to have a previous life in this reality. Now they feel irritable all the time and they don't enjoy things the way they used to. In G2, they expressed suicidal ideas and feelings, were discouraged about the future, perceived themselves as failures more than a person without the disease, and postponed decision-making more than usual. Conclusion: although severe forms of depression have not prevailed, the occurrence of negative symptoms in various spheres of life can evolve, especially in the angolan context.

En G1, la mayoría, además de la tristeza o la infelicidad, no pueden soportar la angustia, algunos verbalizan siendo conscientes de que el llanto no los llevará a tener una vida anterior en esta realidad. Ahora se sienten irritables todo el tiempo y no disfrutan las cosas como antes. En G2 expresaron ideas y sentimientos suicidas, estaban desanimados sobre el futuro, se percibían como fracasados más que una persona sin la enfermedad y postergaban la toma de decisiones más de lo habitual. Conclusión:aunque no han prevalecido formas severas de depresión, la aparición de síntomas negativos en diversas esferas de la vida puede evolucionar, especialmente en el contexto angoleño.

ABSTRACT

A pesquisa objetivou estudar a construção do vínculo mãe-bebê em gestantes soropositivas, a partir da análise dos impactos do diagnóstico para essas mulheres, identificando os sentimentos envolvidos em relação ao bebê e à maternidade, e buscando compreender a vivência da construção do bebê imaginário em gestantes soropositivas. Foi realizada pesquisa transversal, do tipo qualitativa com referencial teórico psicanalítico. Participaram seis mulheres, com idade entre 19 e 36 anos e tempo de gravidez entre quatro e oito meses. Assim, de acordo com o que emergiu a partir das entrevistas, foram estabelecidas as seguintes categorias de análise para Discussão: Conhecimento do diagnóstico, Sentimentos relacionados à gravidez e Construção do vínculo com o bebê. A partir da fala das participantes e das articulações teóricas feitas à luz da psicanálise, foi possível perceber que o vínculo mãe-bebê não é necessariamente prejudicado pela presença do HIV. O desejo de que os filhos fossem saudáveis e a preocupação com relação ao futuro deles esteve presente no discurso das mulheres como importante realidade construída. Além disso, foi possível conceber a existência de um ser, por meio da imaginação e conversa com os bebês, promovendo a aposta para o lugar do sujeito.


This research aimed to study the construction of the mother-baby bond in HIV-positive pregnant women, from the analysis of the impacts of the diagnosis to these women, identifying the feelings involved about the baby and motherhood, and seeking to understand the experience of the construction of the imaginary baby in HIV-positive women. A cross-sectional research was performed in qualitative type and using psychoanalysis as the theoretical framework. Six women participated, aged between 19 and 36 years and time of pregnancy between four to eight months. According to the research aim and the material collected, the following categories were established for discussion: knowledge of the diagnosis, feelings related to pregnancy and construction of the bond with the baby. From the content of what was said by the interviewees and theoretical articulations from psychoanalysis, it was realized that mother-baby bond is not necessarily prejudiced by HIV existence. The desire for their children to be healthy and concerned about the future of them as an important constructed reality. Further, it was possible to conceive the existence of a human being through imagination and talking with babies promoting the bet to subject's place.

Introduction: First case of COVID-19 in the district was reported on 19th March 2020, thereby increas-ing exponentially, presenting with various symptoms to having few or no symptoms, posing challenge to prevent disease transmission by being a significant source of infection. This study conducted to assess relationship between COVID-19 infection within blood groups and burden of reported cases in the dis-trict to seropositivity among donors. Methodology: This is a prospective observational study from second largest blood bank of south Guja-rat, catering to a population of seven million, 4916 donors from the month of August 2020 to May 2021 participated in the study. Observations: Seropositivity was found to be 54.6%, with higher prevalence (68.63%) among female compared to male (54.45%), there was no statistical difference among age groups & RH groups, blood group AB was found to have highest seropositivity followed by A, B and O group. First time donors sero-positivity was higher compared to frequent donors among the study participants. Seropositivity among donors was positively corelated with RTPCR Positivity rate reported in the district. Conclusions: Looking at similar trend of active surveillance and its correlation to blood bank positivity, we urge policy makers to strengthen hospital-based surveillance for real time insights.

Background & objectives: The COVID-19 pandemic has caused significant global morbidity and mortality. As the vaccination was rolled out with prioritization on healthcare workers (HCWs), it was desirable to generate evidence on effectiveness of vaccine in prevailing real-life situation for policy planning. The objective of the study was to evaluate the safety, effectiveness and immunogenicity of COVID-19 vaccination among HCWs in a tertiary care hospital. Methods: This prospective observational study was undertaken on the safety, immunogenicity and effectiveness of the ChAdOx1 nCoV- 19 coronavirus vaccine (Recombinant) during the national vaccine roll out in January-March 2021, in a tertiary care hospital, New Delhi, India. Results: The vaccine was found to be safe, with local pain, fever and headache as the most common adverse events of milder nature which generally lasted for two days. The adverse events following vaccination were lower in the second dose as compared to the first dose. The vaccine was immunogenic, with seropositivity, which was 51 per cent before vaccination, increasing to 77 per cent after single dose and 98 per cent after two doses. Subgroup analysis indicated that those with the past history of COVID-19 attained seropositivity of 98 per cent even with single dose. The incidence of reverse transcription (RT)-PCR positive COVID-19 was significantly lower among vaccinated (11.7%) as compared to unvaccinated (22.2%). Seven cases of moderate COVID-19 needing hospitalization were seen in the unvaccinated and only one such in the vaccinated group. The difference was significant between the fully vaccinated (10.8%) and the partially vaccinated (12.7%). The hazard of COVID-19 infection was higher among male, age >50 yr and clinical role in the hospital. After adjustment for these factors, the hazard of COVID-19 infection among unvaccinated was 2.09 as compared to fully vaccinated. Vaccine effectiveness was 52.2 per cent in HCWs. Interpretation & conclusions: ChAdOx1 nCoV-19 coronavirus vaccine (Recombinant) was safe, immunogenic as well as showed effectiveness against the COVID-19 disease (CTRI/2021/01/030582).

Purpose: Identifying asymptomatic SARS-COV-2 carriage is one of the crucial factors in controlling the COVID 19 pandemic. The relationship between the asymptomatic viral carriage and the rate of seroconversion needs better understanding. The present study was conducted to identify the asymptomatic COVID-19 infection and seropositivity in high-risk contacts in the southern district of Delhi, India. Methods: Following the screening of 6961 subjects, a total of 407 asymptomatic high-risk subjects were selected. Demographic data, socioeconomic status, and history of COVID-19 related symptoms in the last 4 months were recorded. Blood samples and Nasopharyngeal/oropharyngeal swabs were collected for the detection of SARSCOV-2 RNA and anti-SARS-COV-2 antibodies. Results: 55 asymptomatic high-risk subjects (13.5%) tested positive for SARS-COV-2 infection and among them, 70.9% remained asymptomatic throughout their course of infection. The seropositivity among the subjects was 28.9% (n ¼ 118) and was found significantly higher among lower-middle socioeconomic strata (p ¼ 0.01). The antibody levels were significantly higher (p ¼ 0.033) in individuals with a previous history of COVID-19 like symptoms as compared to the subjects, who had no such history. Asymptomatic healthcare workers showed a significantly increased rate of SARS-COV-2 infection (p ¼ 0.004) and seropositivity (p ¼ 0.005) as compared to the non-healthcare workers. Subjects, who were exposed to infection at their workplace (non-hospital setting) had the least RT-PCR positivity rate (p ¼ 0.03). Conclusions: A large proportion of SARS-COV-2 infection remains completely asymptomatic. The rate of asymptomatic carriage and seropositivity is significantly higher in healthcare workers as compared to the general population. The level of SARS-COV-2 antibodies is directly related to the appearance of symptoms. These observations may contribute to redefining COVID 19 screening, infection control, and professional health practice strategies.

ABSTRACT

Purpose: The pandemic of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) might be curtailed by vaccination. We assessed the safety, and immunogenicity of Covishield vaccine among Health care workers (HCWs) in a tertiary cardiac care centre. Methods: It's a prospective analytical study, conducted at Sri Jayadeva Institute of cardiovascular science and research centre, Mysore, between January 2021 to May 2021. Pre and Post vaccination SARS CoV2 IgG antibodies were assessed among 122 HCWs. Interval between two doses in this study were 4 and 6 weeks. Adverse events following immunisation b(AEFI) and efficacy were assessed and followed up for two month post vaccination. Results: Post vaccination seropositivity was 69.67% in overall study participants. Seropositivity and P/N ratio median value in uninfected and infected group were 60.43% (n ¼ 55),3.47 (IQR: 2.56–5.22) and 96.77% (n ¼ 30),9.49 (IQR: 7.57–12.30) respectively (P < 0.001). Seropositivity and P/N ratio after 4 and 6 weeks were 48.3% (n ¼ 60), 2.95 (IQR: 1.91–4.24), and 83.8% (n ¼ 31), 4.88, (IQR: 3.39–6.43) respectively (P < 0.001). AEFI after first and second dose was 72.9% and 27.8% (p < 0.05) respectively. The most common symptoms after both doses of vaccination were local pain (73% & 88.2%), followed by fever (38.2% & 26.5%). The average duration of symptoms in both doses was 1.75 days. Of 122 participants only 10 (8.19%) had breakthrough infection after two doses of vaccination with mild severity. Conclusion: Covishield vaccine has showed seropositivity of 69.67%.It has acceptable level of safety profile. Seropositivity and P/N ratio has increased with increase in interval between two doses. Though it has not prevented breakthrough infection it has certainly reduced the severity of infection.

Purpose: To study the prevalence of human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV) viral seropositive among the patients posted for cataract surgery at a tertiary care center in north India. Methods: It was a cross?sectional study done for 30 months duration. All the patients posted for cataract surgery underwent comprehensive ophthalmic evaluation followed by routine hematological workup, including viral markers for HIV, Hepatitis B surface antigen (HBsAg), and anti?HCV. Data were analyzed by the Statistical Package for Social Science (SPSS Version 20). Results: A total of 7,316 individuals underwent cataract surgery from Jan 2016 to August 2018, 4,073/7,316 (55.7%) were males. The prevalence for HIV was 58/7,316 (0.8%), HBsAg was 151/7,316 (2.1%), and HCV was 11/7,316 (0.1%); 28/58 (48.3%) HIV positives were unaware of their seropositivity till testing, as were 37/151 (24.5%) of HBsAg positives, and 4/11 (36.4%) HCV positives. There was a significant relationship between the mean age in the patients with HIV (P = 0.002) and anti?HCV (P = 0.045). A majority of the seropositive patients were found to be illiterate (45.6%), followed by educated up to high school level (29.1%), and graduate (25.0%). Conclusion: Viral seropositivity was significant among the patients posted for cataract surgery. The eye care providers could refer these patients for counseling and further management for the patient’s and their caretaker’s benefit

Abstract It is debatable whether HIV-infected patients are at greater risk for hepatitis E virus (HEV) infection compared with healthy subjects. The reported anti-HEV seroprevalence among different groups in Bulgaria varied from 9.04% to 25.9%, but the information regarding the HIV population is still missing. The aim of the present study was to evaluate hepatitis E seroprevalence among HIV-infected patients in Bulgaria and to analyze demographic and immunological factors associated with HEV infection. Serum samples of 312 HIV-infected patients were analyzed retrospectively. Age, sex, residence and laboratory markers for HEV, HBV, HCV and HIV infection, and lymphocytes subpopulations were collected for all patients. None of the tested samples were positive for HEV RNA. HEV seroprevalence among HIV-infected patients was 10.9%. Males were more affected with the highest prevalence of positivity in the age group > 30 to ≤ 40 years. The documented HIV transmission routes in HIV/HEV co-infected group were heterosexual, homosexual, intravenous drug use (IDU), and vertical with predominace of the heterosexual route (z = 0.2; p = 0.804). There was a statistically significant trend of HIV mixed infection with routes of HIV transmission other than homosexual - heterosexual in HIV/HEV group and injection drug use in HIV/HBV/HCV co-infected group. The route of HIV transmission, in contexts of patients' behavior, was associated with HEV prevalence among HIV-infected patients.

RESUMO O estudo teve como objetivo discutir sobre os sofrimentos presentes nas narrativas de mulheres que vivem com HIV e os entraves sociais que as colocam em lugares desprivilegiados em suas existências. Trata-se de pesquisa descritiva, exploratória, com abordagem qualitativa a partir do levantamento de narrativas de participantes integrantes do grupo de saúde mental voltado para mulheres que vivem com HIV. Os dados foram analisados percorrendo as fases de pré-análise, exploração do material e tratamento dos dados à luz das autoras feministas Federici e hooks. A partir das narrativas das mulheres, emergiram as seguintes categorias de análise dos dados: o desamparo afetivo e social experimentado após o diagnóstico do HIV; e o sentimento de impotência e de desapropriação do próprio corpo diante da soropositividade. O estudo mostrou os efeitos do estigma associado ao HIV e os sofrimentos que deles decorrem. A partir dos conceitos de solidão e amor, foi possível refletir sobre a necessidade de o cuidado dos profissionais de saúde considerar, além dos aspectos biológicos, a escuta do sofrimento vivido por mulheres com diagnóstico positivo para HIV.


ABSTRACT The study aims to discuss the suffering in the narratives of women living with HIV and the social obstacles that place them in underprivileged places. This descriptive, qualitative and exploratory research was based on the survey of narratives from participant members of the mental health group focused on women living with HIV. The data were analyzed through pre-analysis, material exploration, and data processing in light of the feminist authors Federici and hooks. The following categories of data analysis emerged from the women's narratives: the affective and social helplessness experienced after the HIV diagnosis and impotence and dispossession of one's own body in the face of seropositivity. The study showed the effects of the stigma associated with HIV and the resulting suffering. Based on the concepts of loneliness and love, we could reflect on the need for health professionals' care to consider biological aspects and listen to the suffering of seropositive women.

ABSTRACT Objectives: to analyze the prevalence of the Human Immunodeficiency Virus and the associated factors in pregnant women in the state of Pará. Methods: retrospective, analytical, quantitative study with a sample of 332 medical records of HIV-positive pregnant women hospitalized at the Referral Maternity Hospital in the state of Pará between 2010 and 2019. Bivariate and multivariate statistical analysis were performed with the variables collected. Results: the average prevalence in the period was 2.39% and the Metropolitan Region concentrated 66.87% of cases. There was a strong relationship between the number of antenatal consultations and lack of knowledge of serological status (p value equal to 0.01E-17) variables, and a correlation between the education and number of antenatal consultations variables. Conclusions: the increase in the infection rate during the study period revealed the need to intensify health actions, early diagnosis and strategies to improve adherence to antiretroviral treatment for maternal viral suppression and reduction of the risk of vertical transmission, contributing to improve public policies.


RESUMEN Objetivos: analizar la prevalencia del Virus de la Inmunodeficiencia Humana y los factores asociados en mujeres embarazadas en el estado de Pará. Métodos: estudio retrospectivo, analítico, cuantitativo con una muestra de 332 historias clínicas de gestantes VIH positivas hospitalizadas en la Maternidad de Referencia del estado de Pará entre 2010 y 2019. Se realizó análisis estadístico bivariado y multivariado con las variables recolectadas. Resultados: la prevalencia promedio en el período fue de 2,39% y la Región Metropolitana concentró el 66,87% de los casos. Hubo fuerte relación entre las variables número de consultas prenatales y desconocimiento del estado serológico (valor de p igual a 0,01E-17) y correlación entre las variables educación y número de consultas prenatales. Conclusiones: el aumento de la tasa de infección durante el período de estudio reveló la necesidad de intensificar las acciones de salud, el diagnóstico precoz y las estrategias para mejorar la adherencia al tratamiento antirretroviral para la supresión viral materna y la reducción del riesgo de transmisión vertical, contribuyendo a mejorar las políticas públicas.


RESUMO Objetivos: analisar a prevalência do Vírus da Imunodeficiência Humana e os fatores associados em gestantes no estado do Pará. Métodos: estudo analítico, quantitativo e retrospectivo com a amostra de 332 prontuários de gestantes HIV positivas internadas na Maternidade de Referência do estado do Pará, no período de 2010 a 2019. Com as variáveis coletadas, procedeu-se a análise estatística bivariada e multivariada. Resultados: a média de prevalência no período foi de 2,39% e a Região Metropolitana concentrou 66,87% dos casos. Houve forte relação entre as variáveis número de consultas pré-natais e desconhecimento do status sorológico (p valor igual a 0,01E-17) e correlação entre as variáveis escolaridade com o número de consultas pré-natais. Conclusões: o aumento da taxa de infecção no período estudado revelou a necessidade de intensificar as ações de saúde, o diagnóstico precoce e as estratégias para a melhoria da adesão ao tratamento antirretroviral para supressão viral materna e redução do risco de transmissão vertical, contribuindo para aprimorar as políticas públicas.

Resumen: Objetivo: Analizar la asociación entre la presencia de anticuerpos contra SARS-CoV-2, con la inseguridad alimentaria (IA) y otras características del hogar, en una muestra nacional de hogares mexicanos. Material y métodos: Se obtuvo información a nivel de hogar en una muestra nacional representativa sobre seguridad alimentaria (SA), factores sociodemográficos, cambios en la economía y consumo de alimentos. Se estimó mediante regresión logística ordinal la relación entre IA y características de los hogares y modelos de regresión logística multinomial para estudiar los cambios autorreportados en el consumo de alimentos e IA, ante la presencia de seropositividad contra SARS-CoV-2 en el hogar. Resultados: Los hogares con casos de seropositividad presentaron menor consumo de carnes. En hogares que tuvieron una reducción de sueldo y la presencia de una persona con anticuerpos contra SARS-CoV-2 en el hogar, se presentó un aumento en la prevalencia de niveles más altos de IA (RM=1.225; IC95%: 1.04, 1.44) Conclusiones: La IA en hogares durante la pandemia se agravó por diversos factores, por lo cual se requieren acciones de política pública integrales.


Abstract: Objective: To analyze the association between the presence of anti SARS-CoV-2 antibodies and other household characteristics and the food insecurity in a national sample of Mexican households. Materials and methods: Information at household level about food security (FS), sociodemographic factors, changes in family economy and food consumption was obtained in a national and representative sample. The relation between food insecurity (FI) and household characteristics was established through ordinal logistic regression, multinomial regression models were used to study the self-reported changes in food consumption and FI in the presence of antibodies against SARS-CoV-2 in at least one inhabitant of the household. Results: Households with seropositive cases shown a lower meat consumption. In those households which suffered an income reduction and had at least one individual positive for anti SARS-CoV-2 antibodies, the FI -in its higher levels- worsened (OR=1.225; CI95%: 1.04, 1.44). Conclusions: The food insecurity in households during pandemic worsened due to many factors, in this sense integral actions of public policies are required.

Abstract Background: Bovine leukemia virus (BLV) is the causative agent of enzootic bovine leukosis (EBL). This disease mainly affects cattle, causing severe economic losses to producers. Objective: To establish individual and herd seroprevalence and determine the risk factors associated with BLV seropositivity for dairy and dual-purpose cattle herds in Ecuador. Methods: A total of 2,668 serum samples from 386 herds were collected. A questionnaire, including variables related to cattle health, management and the environment was completed by each herd. A commercial blocking enzyme-linked immunosorbent assay (ELISA) test was used to determine seropositivity. A generalized estimating equation model (GEE) was developed to determine the factors associated with BLV seropositivity. Results: Individual seroprevalence of BLV infection in Ecuador was 17.3% (CI95% = 15.86-18.74%). Herd prevalence was 37.8% (CI95% = 33.0-42.6%), and intra-herd prevalence ranged between 12.5 and 100% (median: 37.5%). The risk factors associated with BLV seropositivity were artificial insemination (OR: 2,215; CI95% =1.402-3.501), concrete floors (OR: 2.178; CI95% = 1.217-3.889), presence of wild ruminants (OR: 2.998; CI95% = 1.788-5.027), and sampling season (wet; OR: 1.996; CI95% = 1.140-3.497). Conclusions: Results indicate that BLV is widespread in cattle herds in Ecuador. In addition, the study suggests that a control program to fight BLV infection should focus on controlling the risk factors identified.


Resumen Antecedentes: El virus de la leucosis bovina (BLV) es el principal agente etiológico causante de la leucosis enzoótica bovina (EBL). Esta enfermedad afecta a los bovinos causando grandes pérdidas económicas a los productores. Objetivo: Establecer la seroprevalencia y dispersión del BLV, así como los factores de riesgo asociados a la seropositividad en explotaciones lecheras y de doble propósito en Ecuador. Métodos: Se recolectó un total de 2.668 muestras de suero de 386 explotaciones. Se aplicó un cuestionario que incluyó variables relacionadas con la salud del hato, medidas de manejo, y características ambientales de cada explotación. Para los análisis serológicos se utilizó un test inmunológico ligado a enzimas (ELISA). Para definir los factores de riesgo asociados a la seropositividad a BLV se desarrolló un modelo utilizando ecuaciones de estimación generalizadas (GEE). Resultados: La seroprevalencia de BLV en Ecuador fue de 17,3% (IC95% = 15,86-18,74%). La dispersión fue de 37,8% (IC95%= 33,0-42,6%), y la prevalencia intra-hato alcanzó rangos entre 12,5-100% (media: 37,5%). Los factores de riesgo asociados a la seropositividad a BLV fueron: inseminación artificial (OR: 2,215; IC95% = 1,402-3,501), piso de concreto (OR: 2,178; IC95% = 1,217-3,889), presencia de rumiantes salvajes (OR: 2,998; IC95% = 1,788-5,027), y temporada de muestreo (húmeda; OR: 1,996; IC95% = 1,140-3,497). Conclusiones: Los resultados indican que el BLV se encuentra disperso en las explotaciones de Ecuador. Adicionalmente, se sugiere la implementación de un programa de control para la lucha contra el BLV, debiéndose considerar medidas que se enfoquen al control de los factores de riesgo identificados en esta investigación.

Resultados: A soroprevalência de BLVno Equador é de 17,3% (IC95% = 15,86-18,74%). La dispersão de 37,8% (IC95% = 33,0-42,6%), e a prevalência intra-rebanho alcançou entre 12,5-100% (media: 37,5%). Os fatores de risco associados à soropositividade a BLV foram inseminação artificial (OR: 2,215; IC95% = 1,402-3,501), chão de concreto (OR: 2,178; IC95% = 1,217-3,889), presença de ruminantes selvagens (OR: 2,998; IC95% = 1,788-5,027) e época da amostragem (úmida; OR: 1,996; IC95% = 1,140-3,497). Conclusões: Os resultados indicam que o BLV se encontra disseminado nas explorações no Equador. Adicionalmente, o estudo pode contribuir para a implementação de um programa de controle para a luta contra o BLV, devendo-se considerar ações de controle dos fatores de risco identificados nesta investigação.

Introducción: la toxoplasmosis adquirida durante por la mujer en edad fértil es un problema aun sin comprender por la comunidad y decisores de los sistemas de salud. En Ecuador existen datos pocos consistentes de la circulación de esta parasitosis, por lo que, ampliar el conocimiento sobre las variables que favorecen su adquisición es de gran interés. Objetivo: determinar la seroprevalencia de la toxoplasmosis en mujeres jóvenes y su relación con variables epidemiológicas. Métodos: Se ensayaron 403 muestras de sueros de estudiantes de la Facultad de Ciencias de la Salud de la Universidad Nacional de Chimborazo, se evaluó el comportamiento serológico de Toxoplasma gondii mediante la determinación de IgG anti Toxoplasma gondii utilizando un método inmunoenzimática, se determinó su relación con hábitos alimentarios, convivencia con animales y conocimiento del riesgo de ser seronegativas al momento de decidir de tener hijos. Resultados: El 26,8 por ciento (108/403) de la muestra resultó seropositivas al Toxoplasma gondii; la vía oral, la forma de adquirir la enfermedad más significativa, se demuestra la relación entre seroprevalencia y convivencia con animales de corral y mascotas. El 73,9 por ciento (298/403) de las encuestadas desconocen que las seronegativas durante el embarazo representan un riesgo potencial. Conclusiones: Se demuestra anticuerpos anti Toxoplasma gondii en sangre de las muestras estudiadas, lo que orienta a infecciones anteriores con Toxoplasma gondii y la influencia de variables estudiadas(AU)


Introduction: Toxoplasmosis acquired by women of childbearing age is a problem still not understood by the community and decision makers from health systems. Few consistent data are available in Ecuador about the circulation of this parasitic infection. Therefore, it is of great interest to broaden knowledge about the variables leading to its acquisition. Objective: Determine the seroprevalence of toxoplasmosis among young women and its relationship to epidemiological variables. Methods: A study was conducted of 403 serum samples from students attending the School of Health Sciences of the National University of Chimborazo. Serological behavior of Toxoplasma gondii was evaluated by determination of anti-Toxoplasma gondii IgG using an enzyme immunoassay. Identification was made of its relationship to eating habits, keeping animals at home, and knowledge about the risk of being seronegative when deciding to have children. Results: Of the total sample, 26.8 percent (108/403) were seropositive for Toxoplasma gondii. The oral route was the most significant mode of acquisition of the disease. A relationship was found between seroprevalence and keeping yard animals and pets. 73.9 percent (298/403) of the respondents were not aware that seronegative results during pregnancy constitute a potential risk. Conclusions: Anti-Toxoplasma gondii antibodies were found in blood of the samples studied, pointing to the existence of previous Toxoplasma gondii infections and the influence of the variables analyzed(AU)

ABSTRACT

Scrub typhus is a vector borne Rickettsial disease caused by Orientia tsutsugamushi and has been reported from tsutsugamushi triangle which includes India. It is a re-emerging pathogen in India including the state of Odisha. This study was conducted to determine seropositivity of scrub typhus in patients attending a tertiary care hospital of Western Odisha.METHODSFifty febrile patients who were negative for Malaria, Dengue and Leptospira attending a tertiary care hospital of Western Odisha were enrolled in the study. 3 ml of blood was collected, and serum was separated. IgM SD Bioline Tsutsugamushi lateral flow immunochromatography test was performed as per manufacturer instruction. Positive samples were confirmed by PCR method (Geno-Sen’s ST Real Time PCR).RESULTS20% of patients were positive for IgM SD Bioline Tsutsugamushi card test. 60% of positive patients were male. 20% of positive patients were having pathognomonic eschar. 60 % of infected patients were in the age group of 10 - 30.CONCLUSIONSIn this study, 20% of febrile patients were positive for Scrub typhus infection. So, it should be included in the differential diagnosis of a febrile patient in Western Odisha.

Objetivo: Analisar a influência da situação socioeconômica e do estágio do vírus da imunodeficiência humana na qualidade de vida de pessoas soropositivas. Métodos: Trata-se de um estudo quantitativo e transversal, realizado entre 2016 e 2017, desenvolvido em um centro de referência no tratamento de pessoas com o vírus da imunodeficiência humana em São Luís (Maranhão, Brasil). A amostra compôs-se de 122 pessoas soropositivas que realizavam tratamento e acompanhamento nesse centro de referência. Utilizou-se um instrumento para coleta de dados socioeconômicos e o questionário de qualidade de vida WHOQol-HIV-bref. A análise dos dados ocorreu por meio da modelagem com equações estruturais. Construíram-se duas variáveis latentes, situação socioeconômica e qualidade de vida, utilizando análise fatorial confirmatória e, em seguida, a análise de caminhos. Resultados: Todas as cargas fatoriais das variáveis latentes apresentaram-se convergentes. Em relação à parte estrutural do modelo, para cada variação de um desvio padrão na situação socioeconômica, há um aumento de 0,601 desvio padrão na qualidade de vida dos participantes. Não houve efeito direto significante do estágio clínico do vírus na qualidade de vida (p=0,165), nem da situação socioeconômica no estágio clínico (p=0,610), bem como não houve efeito indireto da situação socioeconômica na qualidade de vida quando mediado pelo estágio clínico (p=0,654). Conclusão: O estágio clínico não se associou à qualidade de vida e não apresentou efeito mediador entre a situação socioeconômica e a qualidade de vida. A situação socioeconômica influenciou diretamente a qualidade de vida dos participantes.


Objective: To analyze the influence of socioeconomic status and human immunodeficiency virus stage on the quality of life of HIV-positive people. Methods: This quantitative cross-sectional study was conducted between 2016 and 2017 at a reference center for the treatment of people with human immunodeficiency virus in São Luís (Maranhão, Brazil). The sample comprised 122 HIV-positive people undergoing treatment and follow-up at the reference center. We used an instrument to collect socioeconomic data and the WHOQol-HIV-bref. Data analysis was performed using structural equation modeling. Two latent variables were constructed ­ socioeconomic status and quality of life ­ using confirmatory factor analysis followed by path analysis. Results: All factor loadings of the latent variables described converge. With regard to the structural part of the model, for each variation of one standard deviation in the socioeconomic status there was an increase of 0.601 standard deviation in the quality of life of the participants. The clinical stage of the virus had no significant direct effect on quality of life (p=0.165). Also, socioeconomic status had no significant effect on the clinical stage of the virus (p=0.610). Additionally, socioeconomic situation did not have any indirect effect on quality of life mediated by clinical stage (p=0.654). Conclusion: The clinical stage was not associated with quality of life and did not have a mediating effect on socioeconomic status and quality of life. The socioeconomic status directly influenced the participants' quality of life.


Objetivo: Analizar la influencia de la situación socioeconómica y del estado del virus de la inmunodeficiencia humana en la calidad de vida de personas seropositivas. Métodos: Se trata de un estudio cuantitativo y transversal realizado entre 2016 y 2017 desarrollado en un centro de referencia para el tratamiento de personas con el virus de la inmunodeficiencia humana de São Luís (Maranhão, Brasil). La muestra ha sido de 122 personas seropositivas que realizaban el tratamiento y el seguimiento en ese centro de referencia. Se utilizó un instrumento para la recogida de datos socioeconómicos y el cuestionario de calidad de vida WHOQol-VIH-bref. El análisis de los datos se dio a través de ecuaciones estructurales. Se ha construido dos variables latentes, la situación socioeconómica y la calidad de vida, utilizándose el análisis factorial confirmatorio y después el análisis de caminos. Resultados: Todas las cargas factoriales de las variables latentes se presentaron convergentes. Respecto la parte estructural del modelo para cada variación de una desviación típica de la condición socioeconómica hay un aumento de 0,601 desviación típica de la calidad de vida de los participantes. No hubo el efecto directo significativo del estado clínico del virus para la calidad de vida (p=0,165) ni de la situación socioeconómica del estado clínico (p=0,610) así como no hubo el efecto indirecto de la situación socioeconómica en la calidad de vida cuando mediado por el estado clínico (p=0,654). Conclusión: El estado clínico no se asoció con la calidad de vida y no presentó el efecto mediador entre la situación socioeconómica y la calidad de vida. La situación socioeconómica ha influenciado directamente en la calidad de vida de los participantes.

Resumo: Introdução: As Diretrizes Curriculares Nacionais dos cursos de graduação em Medicina preconizam a ênfase na habilidade de comunicação para o profissionalismo do egresso. Promover o desenvolvimento da habilidade de comunicação efetiva para a competência clínica representa um desafio para a educação médica. O presente estudo objetivou contribuir para o desenvolvimento da habilidade de comunicação durante a formação médica por meio de estratégia de ensino e aprendizagem que utilizou como ferramenta o aconselhamento pré-teste HIV no pré-natal. Métodos: Trata-se de estudo com métodos mistos sequenciais e abordagem qualitativa, desenvolvido em três fases. Na primeira fase, 30 estudantes do internato em Tocoginecologia participaram de consulta simulada, gravada em áudio e vídeo, direcionada para conhecimentos específicos e habilidades para comunicação efetiva no tema. Na segunda fase, uma estratégia de feedback individual contou com a análise conjunta do vídeo e avaliação de critérios estabelecidos em checklist específico. Na terceira fase, buscou-se avaliar a autopercepção dos estudantes acerca de sua participação na estratégia e de sua necessidade de desenvolvimento da habilidade de comunicação durante a formação. Resultados: A análise geral dos dados revela um baixo desempenho, sobretudo quanto aos conhecimentos específicos necessários à comunicação efetiva para o aconselhamento pré-teste HIV no pré-natal. Conclusões: Nesse sentido, a autopercepção dos estudantes mostrou-se coerente com os resultados observados e apontou a necessidade de melhorar os conhecimentos específicos aliados às habilidades de comunicação para, então, possibilitar um aconselhamento adequado. Além disso, nos discursos dos estudantes, são frequentes falas reflexivas a respeito da importância de praticar e desenvolver a habilidade de comunicação.


Abstract: Introduction: The Brazilian National Curriculum Guidelines for medical undergraduate courses underscore the emphasis on communication skills for recently graduated professionals. Promoting the development of effective communication skills for clinical competence poses a challenge for medical education. The present study aims to contribute to the development of communication skills during medical education through a teaching and learning strategy that uses prenatal HIV pretest counseling as a tool. Methods: This is a study with sequential mixed methods and qualitative approach developed in three phases. In the first phase, thirty students from the Tocho-Gynecology Internship participated in a simulated consultation, which was recorded in audio and video, directed to specific knowledge and skills for effective communication in the topic. In the second phase, an individual feedback strategy included the joint analysis of the video and the evaluation of established criteria in a specific checklist. In the third phase, the objective was to evaluate the students' self-perception about their participation in the strategy and their need to develop communication skills during medical training. Results: The general analysis of the data reveals a poor performance, especially regarding the specific knowledge required for effective communication for prenatal HIV pretest counseling. Conclusion: Therefore, the students' self-perception was consistent with the observed results and indicated the need to improve specific knowledge combined with communication skills, so as to allow appropriate counseling. Moreover, the students' discourses frequently disclose reflective comments about the importance of practicing and developing communication skills.
